No, the strongest evidence does not show prenatal Tylenol causes autism or ADHD

The argument in brief

The claim that acetaminophen taken during pregnancy causes autism and ADHD is not supported by the best-designed research. Why it spread

The claim carries unusual institutional weight: it was amplified by the U.S. federal government itself when the FDA began a label-change process in September 2025, and then appeared to be validated again when a federal appeals court revived more than 500 lawsuits in July 2026. Both events are easy to read as scientific vindication when they are not — one is a regulatory process explicitly acknowledging that causation has not been established, the other an evidentiary ruling about who may testify. The underlying research also lends itself to misreading: dozens of ordinary observational studies do find small associations, and those headline numbers circulate widely, while the sibling-comparison designs that make those associations disappear require a paragraph of explanation. Add a highly emotive subject — pregnancy, autism, a household drug taken by a majority of pregnant women — and the claim travels far faster than the correction.

The claim resurfaced after the FDA moved on September 22, 2025 to begin changing acetaminophen's label to describe a possible link to neurodevelopmental disorders, and again after a federal appeals court revived hundreds of Tylenol lawsuits on July 13, 2026. Judged against the best-designed studies now available, the causal assertion is not supported. It rates FALSE.

The decisive evidence comes from sibling comparisons, which hold constant the genetics and household environment siblings share. In JAMA in April 2024, Ahlqvist and colleagues analyzed 2,480,797 children born in Sweden between July 1995 and December 2019. Conventional models produced the small elevations that fuel the claim: autism hazard ratio 1.05 (95% CI 1.02-1.08), ADHD 1.07 (1.05-1.10). Among 1,773,747 full siblings, those signals vanished — autism 0.98 (0.93-1.04), ADHD 0.98 (0.94-1.02), intellectual disability 1.01 (0.92-1.10). The authors concluded the earlier associations likely reflected confounding.

That result has been replicated in a different population. On June 29, 2026, JAMA Internal Medicine published a study co-led by the University of Hong Kong and Aston University, drawing on Hong Kong health records from 2001 to 2023: 708,020 mother-child pairs, 43.3% exposed to paracetamol in utero. In sibling-matched cohorts of 124,333 children for autism and 97,285 for ADHD, the adjusted hazard ratios were 1.00 (0.91-1.11) and 1.01 (0.93-1.08). HKU reported the null result held regardless of dose, trimester, or whether use was sporadic, intermittent or persistent.

An association in the general population that disappears within families is the signature of confounding rather than causation. Women who take acetaminophen in pregnancy differ systematically from those who do not — more infections, fevers, chronic pain and psychiatric diagnoses, several of them heritable and independently linked to neurodevelopment. A systematic review by D'Antonio and colleagues, published January 16, 2026 in The Lancet Obstetrics, Gynaecology, & Women's Health, screened 43 studies and pooled 17. Restricted to studies at low risk of bias, pooled odds ratios were 1.03 (0.86-1.23) for autism, 0.97 (0.89-1.05) for ADHD and 1.11 (0.92-1.34) for intellectual disability.

The claim is not baseless. In August 2025, in the journal Environmental Health, Diddier Prada, Andrea Baccarelli and colleagues applied the Navigation Guide method to 46 studies: 27 reported significant associations, nine found none and four suggested protective effects, with the strongest signal for use of four weeks or longer. They urged caution while stopping short of asserting causation. Readers should know the overlap between that evidence and the litigation: Baccarelli, dean of Harvard's Chan School, disclosed in the paper's competing-interests section that he served as an expert witness for plaintiffs, and he is one of the three experts the appeals court reinstated.

That ruling revived more than 500 suits against Kenvue, holding that Judge Denise Cote wrongly excluded testimony which, Judge Guido Calabresi wrote, "constitute acceptable interpretations of scientific evidence where scientists may, and in fact do, disagree." Reuters reported that Calabresi stressed the court was not deciding whether acetaminophen causes autism or ADHD. Admissibility means a jury may hear an expert, not that the expert is right. Clinical guidance has not moved: ACOG's September 22, 2025 practice advisory calls acetaminophen the safest first-line analgesic and antipyretic in pregnancy, and the Society for Maternal-Fetal Medicine — whose president Sindhu K. Srinivas called the causal evidence "simply inconclusive" in September 2025 — reaffirmed that position on June 1, 2026.

Regulators have not formally closed the question. The FDA's own September 2025 notice to physicians conceded that "a causal relationship has not been established" and that contrary studies exist, and its label-change process remains unresolved; Kenvue's May 27, 2026 label update carries no autism warning, and the company has asked the FDA to reject the requested change. The alternatives are worse: SMFM's June 2026 guidance advises against NSAIDs and opioids as first-line in pregnancy, citing fetal renal impairment and premature closure of the ductus arteriosus. Residual uncertainty remains — but the claim as stated, that Tylenol causes autism and ADHD, outruns the evidence badly.
