No — measles is far more dangerous than the measles vaccine, by roughly a thousandfold on brain inflammation alone

The argument in brief
The claim that MMR vaccination is more dangerous than catching measles is false. Encephalitis follows roughly 1 in 1,000 measles infections but about 1 in 1,000,000 MMR doses, according to the StatPearls MMR monograph, and measles killed 400 to 500 Americans a year before the vaccine — while the Infectious Diseases Society of America states there have been no deaths shown to be related to the MMR vaccine in healthy people.

Why it spread
The claim survives because it exploits a real asymmetry in visibility. Vaccine side effects are administered on a known date to a healthy child, so any bad outcome that follows has an obvious candidate cause; measles complications are diffuse, delayed, or simply absent from living memory because vaccination removed them. Two generations of American parents have never seen a measles encephalitis case, which makes the disease feel hypothetical and the injection feel concrete. The framing also travels well politically — KFF found roughly twice the belief rate among Republicans and independents as among Democrats — and gains traction whenever a genuine vaccine risk (febrile seizures, anaphylaxis, contraindication for the immunocompromised) is real enough to cite but small enough that its true magnitude goes unstated. The comparison then quietly drops the denominator on both sides.
The claim that getting the measles vaccine is more dangerous than being infected with measles is false, and it is not a close call. On every outcome that has actually been measured — death, brain inflammation, hospitalization, long-term immune damage — the disease is more dangerous than the shot, usually by orders of magnitude.
Start with the disease. In the decade before the first measles vaccine was licensed in 1963, the United States saw an estimated 3 to 4 million measles cases a year, causing 48,000 hospitalizations, 1,000 cases of encephalitis and 400 to 500 deaths annually, according to the Illinois Department of Public Health's history of the disease. Those risks did not vanish; the vaccine simply removed most of the exposure. The Infectious Diseases Society of America still puts brain swelling at about 1 in 1,000 measles cases, pneumonia at as many as 1 in 20, and death at nearly 3 in 1,000, with complications of some kind in 3 in 10 people who catch it.
Now the vaccine. The cleanest apples-to-apples comparison is encephalitis: the StatPearls MMR monograph on NCBI Bookshelf, updated May 2025, reports that post-vaccination encephalitis "occurs in approximately 1 in 1 million doses, compared to 1 in 1000 with infection by the measles virus." The same monograph puts malaise and fever at about 5% of immunized children, notes that vaccine-associated low platelet counts usually resolve within three months, and gives two-dose effectiveness against measles as 99%. IDSA states flatly that there have been no deaths shown to be related to the MMR vaccine in healthy people.
The claim deserves a fair hearing on one narrow point: MMR is not risk-free, and it is a live attenuated vaccine. The Institute of Medicine's 2012 review, Adverse Effects of Vaccines: Evidence and Causality, concluded that the evidence "convincingly supports" a causal relationship between MMR and febrile seizures, anaphylaxis, and — in people with demonstrated immunodeficiencies — measles inclusion body encephalitis. Anaphylaxis is not a trivial reaction, and the encephalitis finding is exactly why MMR is contraindicated for the severely immunocompromised; IDSA notes that the rare vaccine-related deaths on record occurred among immunocompromised children. That same IOM review found the evidence "favors rejection" of a causal link between MMR and autism, and between MMR and type 1 diabetes.
None of that supports the comparative claim. The American Hospital Association, citing CDC, reported on July 24, 2026 that 2,318 confirmed measles cases had been recorded in the U.S. this year — the most since 1991 — with cases reported by 45 jurisdictions, the vaccination status of 93% of cases unvaccinated or unknown, and 7% hospitalized. In 2025, three people died of measles in the U.S., all unvaccinated, according to a review in the International Journal of Surgery; WHO reported at the time that the first of those was the first U.S. measles death in a decade. Globally, WHO's July 2026 fact sheet estimates 95,000 measles deaths in 2024 and credits measles vaccination with averting nearly 59 million deaths between 2000 and 2024.
Measles also harms people who survive it. A 2015 Science study led by Michael Mina inferred from population-level mortality data that measles elevates deaths from unrelated infections for two to three years afterward. A 2019 Science study by Mina and colleagues then measured the mechanism directly: in 77 unvaccinated children, measles eliminated 11% to 73% of the existing antibody repertoire — an effect the authors reported was "not observed in infants vaccinated against MMR."
The polling explains why this matters now. KFF's tracking poll of 2,480 adults, fielded May 7–31, 2026 and published June 30, found 29% had heard the claim. An earlier KFF poll in April 2025 found 20% of Republicans and 21% of independents called it definitely or probably true, against 11% of Democrats. It circulates in a country where kindergarten MMR coverage fell to 92.5% in 2024–25, below the 95% needed to interrupt transmission, CIDRAP reported from CDC data.
